RELIGIOUS EDUCATION REGISTRATION FORM

 YOUTH GROUP, CONTINUATION, and KIDS CLUB WILL BE ON WEDNESDAYS-7PM-8PM (STARTING AUGUST 15TH)


All Students and family for all classes are asked to attend August 15, 2018, Solemnity Mass, Solemnity of the Assumption of the Blessed Virgin Mary.  This is the kick-off for 2018-19 Religious Education year.  


All Students for all classes will begin instruction on Sunday, August 19th from 1:30-2:30 PM.

Sacrament Preparation

The preparation for the Sacraments of First Reconciliation, First Eucharist and Confirmation is an active event in the lives of our parish children and their families. It is a participator process and we offer required sessions for parents to enable them to fully support their children throughout this time.


Sacramental Preparation Registration Criteria

• There are three steps to finalizing enrollment in Religious

  Education/Sacrament Preparation.

• Children age 7 and up, or turning 7 before Easter 2019 may register

  for our First Reconciliation and First Communion classes.

• In addition to Sacrament Preparation classes, children are expected

  to attend Mass and Holy Days of Obligation as required by the 3rd

  Commandment.

• Consistent attendance in their Religious Education during the year of

  Sacrament preparation is necessary to ensure a proper foundation for

  Sacrament preparation.

• Student who are developmentally challenged or who have learning

  disabilities are welcome in the formation process. Particular care

  and understanding of their needs and situation are important to our

  Sacrament preparation team. Parents/guardians are advised to meet

  with Josh Mangels to discuss how these children can participate in

  the Sacramental and faith life of our church community.

• Any child transferring from another parish must provide

  documentation from the Pastor or his delegate showing that they

  meet basic requirements.

• Parents/guardians are to be registered members of San Martin de

  Porres.

• Every parent/guardian must schedule(in this form) and complete an

  Enrollment Interview before registration is complete.

I authorize my child, in whose care of the minor has been entrusted by the Roman Catholic Parish of San Martin de Porres-Sahuarita, to consent to any X-ray examination, anesthetic, medical, surgical, or dental diagnosis of treatment and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any licensed physician or dentist.  I agree to be liable and to pay all the costs and expenses incurred in connection with such medical and dental services rendered.  Should it be necessary for our (my) son/daughter to return home due to medical reasons or otherwise, I agree to assume all transportation costs.

AGREEMENT

For parents or legal guardian of students enrolled in our Religious Education Program


As parent(s) or legal guardian(s) of our children attending Religious Education at San Martin de Porres we agree that...


1. We send our children to receive catechism on our own free will. AGREE:Yes  No


2. As Catholics, we must attend Mass every Sunday either Saturday Vigil evening or on Sunday as well as all Holy Days of Obligation. (Weddings, funerals or quinceañera masses do not fulfill the obligation to go to Mass on Sundays). The child will have his Mass Attendance Book stamped/ signed after mass in the Faith Formation or Parish Office.

      • Attendance requirement is based on a point system. 144 points

        are required to complete the religious education year. 3 points

        are given each
        Sunday and 2 points each Wednesday (kids club and youth

        group).

      • If child attends Sunday Mass elsewhere he/she must have their

        mass attendance book or bulletin signed by the priest celebrant

        of that mass.

      • Regular attendance at another parish may result in

        postponing the sacraments or requiring the child to register

        elsewhere.

3. The child must attend all scheduled classes as stated on the Calendar. Consistent attendance in their Religious Education during the year of Sacrament preparation is necessary to ensure a proper foundation for sacrament preparation.  Please schedule all your other obligations accordingly.

4. The child must dress appropriately for classes, spiritual gatherings and other activities. He/she cannot wear “short shorts,” miniskirts, spaghetti straps, tank tops, backless blouses, midriff bearing or low cut shirts.  Nor can he/she wear baggy shorts, pants or shirts. Hats, baseball caps, and expensive jewelry are not allowed either.

5. The child must turn in all required homework assignments as requested by the instructor on the day it is due. Students must bring class materials with them to each class.

6. Parent(s)/guardian(s) are responsible for adhering to the Drop Off and Pick Up policy of San Martín de Porres.

      • IF THE CHILD IS 12 YEARS OR YOUNGER HE/SHE MUST BE

        SIGNED OFF BY AN ADULT no later than 10 minutes after class

        ends.

      • NO CHILD MAY BE DROPPED OFF UNTIL THE TEACHER OR

        A SUBSTITUTE HAS ARRIVED.

      • I/we understand that if a child is not properly signed in and out it

        may result in an absence.

      • Parent(s) and/or guardian(s) are legally responsible for the

        child’s whereabouts at all times.

      • If pick up takes longer than 20 minutes after classes end, the

        faith formation program will call the police and/or Child Protective

        Services for your child’s protection.

      • If anyone other than the parent is to pick the child up after class,

        the office must be notified prior to the beginning of class..

9. I/we must turn in all sacramental records and other required paperwork to the faith formation office during Parent Intake Interview (Step 3), otherwise, the reception of a sacrament might be postponed.

I/we will check the child’s folder for notes so as to be informed of upcoming events, meetings or schedule changes.  I/we will take responsibility for sending a responsible adult in my place if I cannot attend a meeting. All scheduled meetings are required for all parents in order to be better informed of my child’s faith journey. I understand that makeup meetings will not be offered.
